On 06/18/04, strained back at work & experienced intense pain when moving or while stationary. Lower back pain associated with herniated disc is L5 - S1. Also have a slight disc tear in L2 - L3. Confirmed with MRI\'s, Flexion Extension X-Rays a DiscoGram and a CAT Scan. Performed at the Barrow Insitute for Neurological Surgery in Phoenix. Also completed a series of 3 epidural blocks. Any surgical intervention has been held off while condition continues to slowly improve. Condition also is responsive to physical therapy, traction, etc.
